Investigating An Accident
The following instructions can help you gather the necessary information to ensure a claim is handled quickly and fairly.
Employee Incident
- If employee is unconscious, call for medical assistance (911).
- If employee is conscious determine whether or not to summon medical assistance.
- Secure the accident scene.
- Interview employees and document their statement.
- Indentify witnesses, including employees, who were in the area.
- Take photos, or draw a diagram of accident area.
- Report claim to management and insurance company.
Customer Incident
- If customer is unconscious, call for medical assistance (911).
- If customer is conscious determine whether or not to summon medical assistance.
- Secure the accident scene.
- Gather customers personal information (name, address, phone, etc.).
- Identify witnesses, including employees, who were in the area.
- Conduct on-site investigation.
- Make notes of any conditions in the floor, sidewalk, parking lot, etc., or any unusual conditions (lack of marks through liquid, etc.).
- Complete your Customer Incident Report.
- Report claim to management and insurance company.
