Investigating An Accident

The following instructions can help you gather the necessary information to ensure a claim is handled quickly and fairly.

Employee Incident

  • If employee is unconscious, call for medical assistance (911).
  • If employee is conscious determine whether or not to summon medical assistance.
  • Secure the accident scene.
  • Interview employees and document their statement.
  • Indentify witnesses, including employees, who were in the area.
  • Take photos, or draw a diagram of accident area.
  • Report claim to management and insurance company.
Customer Incident
  • If customer is unconscious, call for medical assistance (911).
  • If customer is conscious determine whether or not to summon medical assistance.
  • Secure the accident scene.
  • Gather customer’s personal information (name, address, phone, etc.).
  • Identify witnesses, including employees, who were in the area.
  • Conduct on-site investigation.
  • Make notes of any conditions in the floor, sidewalk, parking lot, etc., or any unusual conditions (lack of marks through liquid, etc.).
  • Complete your Customer Incident Report.
  • Report claim to management and insurance company.
